The decision that sinks a paper is made in week one, and the symptoms do not show up until month four, when the time is already spent. This is how to tell in an evening whether a project will reach submission.

The discussion is not a summary. It is the argument, and knowing that changes what you write and how long it takes.
Picking the journal after the paper is written is one of the most common reasons for a desk rejection. The doctors who get accepted decide much earlier, and it changes everything before it.
